Two use cases for FHIR’s impact on population health

One such case is the scenario of providers using an EMR as their main workflow tool, but needing population health alerts for their patients. Because accurate alerts require a complete and precise longitudinal chart, EMRs simply do not have the data to offer that functionality.

A second use case for FHIR and population health revolves around care plans and care management. Because population health management tools generally have more complete and accurate data, they serve as better tools for care managers who need the complete picture of a patient’s health in order to make informed decisions about care plans.

Population health vendor Wellcentive offers some interesting use use cases of FHIR that will allow caregivers to have a more complete view of a patient’s medical record than what is available in an EHR alone. Great scenarios that both require interoperability between multiple applications.
