The customer

Monogram Health

Monogram Health is a value-based provider of in-home, evidence-based care and technology-driven care management services for people with polychronic conditions, notably chronic kidney disease. Their integrated model combines purpose-built technology with a team of interdisciplinary specialists, including cardiologists, endocrinologists, nephrologists, social workers, nurse practitioners, and behavioral health therapists. This wraparound care approach enables Monogram to support patients holistically, emphasizing conservative and palliative care options while improving access to affordable care in 36 states.

Monogram’s application development team, made up of developers, administrators, and analysts, is focused primarily on building the tools their clinical providers use daily. From charting patient encounters to surfacing actionable insights, their applications help frontline teams know when and how to intervene — empowering timely, data-driven care that improves outcomes.

The challenge

Scaling data infrastructure while maintaining complete patient records

Monogram collects patient data from many sources: HIEs, payers, e-prescribers, labs, and internal systems. Inconsistent formats and data quality created hundreds of duplicate records each month — adding to the dev team’s burden and creating an integration bottleneck as Monogram added new sources and partners. The problem worsened when patients switched payers or providers, particularly after annual open enrollment.

This poor data quality required manual, expensive reconciliation. Monogram’s infrastructure at the time lacked the tools necessary to efficiently resolve identity and data integrity issues across its growing partner ecosystem.

As the development team was pulled further into maintaining infrastructure, it became harder to prioritize high-impact work such as streamlining onboarding, optimizing workflows, and improving the clinical experience.

Monogram needed a partner that could:

  • Simplify integration workflows and maintenance
  • Speed up onboarding for new payers and partners
  • Support digital engagement through Salesforce Health Cloud
  • Free up technical resources to focus on innovation

“We didn’t want to own all the integration work internally. It’s necessary, but it’s not where we uniquely add value… We had to ask ourselves: do we want to maintain our internal software and all the downstream challenges (duplications, processes)? Who can support that? Rhapsody has that expertise.”

VP of Application Development, Monogram Health

The solution

Simplifying complexity to scale smarter

Monogram chose to consolidate integration efforts with Rhapsody’s Envoy iPaaS, embracing a buy-not-build strategy. With prebuilt API connectors and proactive monitoring, With prebuilt API connections and proactive monitoring, Envoy simplifies integration across EHRs, Salesforce Health Cloud, and other systems at scale — reducing manual work and freeing up three developers to focus on more strategic initiatives.

Instead of dedicating headcount to interface maintenance, Monogram’s developers now focus on building differentiated workflows that enhance both the clinical and patient experience.

“We look to buy wherever we can accelerate features to market, utilize expertise we do not have, or scale in ways that would take us too long to build internally.”

Cameron Kerber

VP of Application Development, Monogram Health

Envoy also made it easier for Monogram to expand its interoperability strategy over time. The team began onboarding Rhapsody EMPI to enhance identity resolution, with confidence it could scale up gradually.

At every step, Kerber’s team trusted their Rhapsody partners. “I would always recommend Rhapsody,” Kerber said. “Ultimately, when you’re working with these types of partners, it comes down to the people. They’re great. They’re super responsive. They’re smart. We haven’t had turnover.”

The results

Speed, scale, and strategic impact

50% Faster Onboarding
Monogram now brings on new payer and partner data sources 50% faster using Rhapsody Envoy and best practices. With seamless integration into platforms like Salesforce Health Cloud and Athena APIs, the team has built workflows that elevate the clinical and patient experience.

Smarter Identity Resolution
Even as new sources introduce thousands of records, Monogram ingests data quickly and accurately. The rollout of Rhapsody EMPI is already reducing duplicate cleanup and improving engagement by ensuring clean, matched person data from the start.

$600K in Annual Development Savings
By shifting from manual interface maintenance to scalable, prebuilt integrations, Monogram has saved an estimated $600,000 annually. Three developers have been reallocated to focus on innovation and strategic projects.

“Envoy lets us scale without adding headcount. We can plug in new HIEs or partners quickly using what we’ve already built. It just works — and that means we can focus on more strategic initiatives. “
