Monogram Health
Monogram Health is a value-based provider of in-home, evidence-based care and technology-driven care management services for people with polychronic conditions, notably chronic kidney disease. Their integrated model combines purpose-built technology with a team of interdisciplinary specialists, including cardiologists, endocrinologists, nephrologists, social workers, nurse practitioners, and behavioral health therapists. This wraparound care approach enables Monogram to support patients holistically, emphasizing conservative and palliative care options while improving access to affordable care in 36 states.
Monogram’s application development team, made up of developers, administrators, and analysts, is focused primarily on building the tools their clinical providers use daily. From charting patient encounters to surfacing actionable insights, their applications help frontline teams know when and how to intervene — empowering timely, data-driven care that improves outcomes.
The challenge
Scaling data infrastructure while maintaining complete patient records
Monogram collects patient data from many sources: HIEs, payers, e-prescribers, labs, and internal systems. Inconsistent formats and data quality created hundreds of duplicate records each month — adding to the dev team’s burden and creating an integration bottleneck as Monogram added new sources and partners. The problem worsened when patients switched payers or providers, particularly after annual open enrollment.
This poor data quality required manual, expensive reconciliation. Monogram’s infrastructure at the time lacked the tools necessary to efficiently resolve identity and data integrity issues across its growing partner ecosystem.
As the development team was pulled further into maintaining infrastructure, it became harder to prioritize high-impact work such as streamlining onboarding, optimizing workflows, and improving the clinical experience.

Monogram chose to consolidate integration efforts with Rhapsody’s Envoy iPaaS, embracing a buy-not-build strategy. Envoy also made it easier for Monogram to expand its interoperability strategy over time. The team began onboarding Rhapsody EMPI to enhance identity resolution, with confidence it could scale up gradually.
